A teen member with diabetes was having difficulty keeping their blood sugar at a healthy level.  A Member Connections representative from Coordinated Care, went to visit this member at their home following a hospital stay for a diabetic crisis. The staff member met with the patient and their mother to review some educational materials about diabetes and the directions established by our Case Manager. The mother was very thankful for our case management and for the Nurtur Disease coaching program for her child and said she wished we had been here as company 6 years ago when her child was diagnosed with Diabetes.

Coordinated Care is continuing to assist this family through Case Management. The family has been helped to sign up for a Safelink phone so they could reach case management, our 24-hour Nurse Line or their doctor anytime. The teen is doing much better in monitoring their blood sugar and taking insulin shots.

This is another example of how we help people get well, stay well and be well.

A member sent a warm ‘Thank You’ to everyone she interacted with, including Joyce and Bridget for their empathy, compassion, and care. Ron, for reaching out to Case Management /Behavioral Case Management and Christopher for assisting with getting her prescriptions filled. This member was emotionally distressed and scared.  She wanted to share with us that throughout her many years battling many  serious conditions, she has never been treated with more respect and has never had as quick of a response from anyone.

The CLE session, titled “The Ins and Outs of the Affordable Care Act,” will provide attorneys in Washington, 3.5 CLE credits for attendance.  Dr. Fathi’s expertise in Washington as a provider, patient advocate and now as a Medicaid payor, gives a unique and balanced perspective to the benefits of managed care.

Dr. West, a graduate of the University of Washington School of Medicine, completed a residency in Family Medicine with the Providence St. Peter Residency Program. Dr. West has been practicing Family Medicine in Washington his entire career and will continue to see patients one day a week at Edmonds Family Medicine/Puget Sound Family Physicians where he has practiced for the last three years. Previously, Dr. West cared for patients at the Polyclinic/Ballard and Chief of Staff at Ballard Swedish Hospital.

Dr. West currently serves as co-chair of Government Affairs for the Washington Academy of Family Physicians and is on the Committee for Quality and Practice for the American Academy of Family Physicians.

FOR IMMEDIATE RELEASE. (Tacoma, WA) January 29, 2013. Pierce County has more than 10,000 children who qualify for Washington’s State health programs and are not currently enrolled. That’s a number that is just not acceptable to those who are coordinating a series of 7 family-oriented enrollment fairs throughout Pierce County.  The series, titled the Carousel Enrollment Fairs, begin on Feb. 9^th and will offer fun and family-oriented entertainment, prizes, information on health care and most importantly, on-site enrollment for these children and their families.

“This is going to be a free and fun opportunity for families who need it most,” states Rudy Vasquez,

Multicultural Services Director for Sea Mar Community Health Centers. “Our goal is that the families can come and enjoy some fun, but walk away having had an opportunity to be connected to important community services and benefits such as health care coverage, food and other resources as needed.”

The Washington State Coalition for Healthy Communities of Color, the coordinating entity, is bringing together community groups, agencies, the state’s five Medicaid health plan providers and the Department of Social and Health Services (DSHS) Mobile CSO enrollment truck to ensure that families get the information and services needed. Bilingual staff will be available.

Pierce County is the first “stage” location of what might become a multi-county outreach effort if all goes well with this first attempt.  The Carousel Health Fairs begin on Feb. 9^th in Fife and will rotate to varying sites in Pierce County through May.  DSHS, Sea Mar Community Health Centers, Community Health Care Tacoma, Comprehensive Health Education Foundation, Amerigroup, Community Health Plan of Washington, Coordinated Care, Molina Healthcare, United Healthcare, WithinReach, Mercy Housing, WIC offices and many more agencies will be present to provide free activities, prizes and information to families.
