Liaison Contact Information
Apple Health Core Connections Liaisons are assigned to each region to make certain that Department of Children, Youth and Families (DCYF) staff have on-going support in the program. Liaisons are focused on effectively promoting communication between DCYF, Coordinated Care, members, caregivers and healthcare providers. (Regions are based on Department of Children, Youth and Families Regional Structure (PDF)).
