STEP 1 Complete a healthy activity.
STEP 2 Reward dollars will be put on your CentAccount® Rewards Card after we receive the claim for your healthy activity.
STEP 3 If it’s your first reward, a card will be mailed to you.
STEP 4 Spend your reward dollars at one of the following participating stores:

*Many OTC medicines and supplies are a benefit at no cost when prescribed by your doctor.  Or, for your convenience, you may buy them with your CentAccount Healthy Rewards card.

For a complete list of items you can buy, log in to your member account. Remember, this card is not a credit card. It already has funds on it. Keep the card after you use it. As you earn more rewards, they will be added to this card.



$30 For 6 infant well care visits with assigned primary care doctor before 15 months old. (Must be with assigned primary care doctor.)
$10 For an annual well care visit with assigned primary care doctor. (One per calendar year; age 2 and up)
$10 For an annual cervical cancer screening. (One per calendar year; females age 21-64.)
$10 For an annual breast cancer screening. (One per calendar year; females age 50-74.)
$50 For annual comprehensive diabetes care – must complete all of the
following once in the calendar year (age 18-75):

  • HbA1c test
  • LDL-cholesterol test
  • Kidney screening
  • Retinopathy screening (dilated eye exam)

Frequently Asked Questions

What is the CentAccount card?  The CentAccount card is a pre-paid gift card from Coordinated Care to reward your choice to take control of your health and perform important preventive health actions.

Who can get a CentAccount card? All Coordinated Care members are eligible for a CentAccount card.

Can members under 18 get a CentAccount card? Yes. The card will be issued in the child’s name and sent to the parent or guardian of the member who is a child.

When do I get my CentAccount card? You will get a CentAccount card after you complete at least one of the healthy activities that earn a reward. The card will be mailed to your home, pre-loaded with the reward amount earned already on it. You should keep this card because all future rewards will be deposited on this same card in the future.

I’ve completed a healthy activity to earn a CentAccount reward but I still have not received my card or the incentive amount. Should I be concerned? Rewards are applied to cards based on claims submitted by your provider. It usually takes a few weeks to have the money applied to your card.

Who do I call if I haven’t received a card/reward? If you have completed a qualifying healthy activity and would like to check the status of your card or reward, please call Member Services at 877-644-4613.

How do I get a new card if it is lost or stolen? Please call Member Services at 877-644-4613 to report your lost or stolen card.

Can I get more than one reward during one doctor visit? Yes! Please talk with your doctor about setting up an appointment that completes one or more of the healthy activities. You will receive a reward for each qualifying action, even if they are in the same appointment.

How often can I get a reward for seeing my doctor? You will receive one reward per calendar year for check-ups and health screenings for children or adults. For example, if you have already earned a reward for a yearly check-up in 2014, you will get a reward for a annual health check up at your first appointment in the year 2015.

How long will it take to have a reward put onto my card after I complete a healthy behavior? Rewards can take several weeks to appear on your CentAccount card. If you have questions, call Member Services at 877-644-4613.

Can I use my CentAccount card at an ATM? No. The CentAccount card will not work at an ATM. Your CentAccount rewards can not be redeemed for cash.

Will my CentAccount card expire? Yes. Your card can expire in three ways:

  1. One year from the date of your first reward.
  2. After 60 days of your last date of coverage with Coordinated Care.
  3. If the card is inactive (no additional rewards or transactions) for 12 months.
