Coordinated Care Harvest BucksTM: Making fresh fruits & vegetables more accessible to members.

The fruit and vegetable prescription program is a benefit offered by Coordinated Care to members who want to increase the amount of fruits and vegetables in their meals, want to try new fruits and vegetables, or are experiencing food insecurity.

Coordinated Care is partnering with the Washington State Department of Health and Safeway to offer this benefit to members.

We are the first Apple Health (Medicaid) plan in Washington to offer a food benefit to members.

There are two ways to receive Coordinated Care Harvest BucksTM prescription vouchers:

Clinic Referrals

We collaborate with local clinics to expand the distribution of Coordinated Care Harvest BucksTM vouchers to members. Vouchers are prescribed to eligible members through our participating clinics:

  • Sea Mar Community Health Center clinics
  • Columbia Valley Community Health Centers
  • CHAS Health (Community Health Association of Spokane)

In addition, members may receive nutrition education, recipes, social support through classes, and one-on-one visits with nutritionists, social workers, health educators, and community health workers with this program at participating providers.

Self-Referrals

Members may self-refer to request Coordinated Care Harvest BucksTM
vouchers by emailing foodRx@CoordinatedCareHealth.com or calling member services at 1-877-644-4613 (TTY: 711)

 

Frequently Asked Questions

Vouchers are valued at $10 each and can be used at any Safeway stores to buy fruits and vegetables that are fresh, canned (no added sugar or salt), and frozen (no added sauces or fried potatoes).

All Apple Health (Medicaid) Coordinated Care members are eligible for the program. Members must have active coverage with Coordinated Care at the time of issuance. Both adults and children qualify, as long as they are actively enrolled with Coordinated Care.

Yes, eligible Coordinated Care members can participate in Coordinated Care Harvest BucksTM if they receive food benefits or participate in other food programs.

Shop at any Safeway store in Washington (don’t forget to enter your Safeway club card number/phone number)

Spend the Full Amount

  • The total voucher must be used in one purchase; change is not given for spending less than $10. If your total is equal to or more than your Coordinated Care Harvest BucksTM voucher you will spend the full amount. You will need to pay any extra amount.

Checking Out

  • You must go to a checkout lane with a cashier. Do not use “self-checkout;” self-checkout machines cannot process the vouchers.

Yes, you can use more than one Coordinated Care Harvest BucksTM vouchers in a single purchase. Remember, change is not given for spending less than the total amount of vouchers.
