Case Management

Sometimes getting the care you need isn’t always a clear path. Our case managers are here to help. They can help you get the care you need and understand your options.

We will work with you to see if you are eligible to have a case manager. Members with chronic conditions, such as diabetes or asthma, are typically well-suited for this program. You can refer yourself or be referred by a provider. You can also refer friends and family.

Case managers can help with:

  • Finding providers and specialists to give you the care you need
  • Accessing benefits like a SafeLink phone
  • Using nearby resources to improve your health
