Did you leave your Member ID card at home? Do you want to view your rewards while shopping at the grocery store? The Mobile App allows you to do all of this, and much more.

It’s easy to download! Just search for “Coordinated Care” on your Apple or Android phone or tablet through the App store. Make sure you know your online member login information as you will need access your account.
